HealthPartners
® Freedom plans are Medicare Cost plans, that pick up where Medicare
coverage leaves off. It has an open access network which means you can
obtain services from any of the HealthPartners contracted providers that
include most doctors and hospitals throughout Minnesota. HealthPartners is a health plan with a Medicare contract.
There are three medical plans with drug options:
- Plan I - most affordable
- Plan II - most popular
- Plan III - most comprehensive

How to Enroll
Complete the HealthPartners® Freedom Enrollment Form (Medical Only)
plus HealthPartners® Freedom Prescription Drug Enrollment Form
and mail or fax it to HealthPartners. Please read this information prior to enrolling. For potential for contract termination and members' rights and responsibilities upon disenrollment, please refer to your Evidence of Coverage Section 13.
Travel Coverage
With HealthPartners® Freedom plan, you can take your health plan with you when you travel. HealthPartners ® Freedom plan offers an Extended Absence benefit at no additional cost.
